Compounded Tirzepatide

Dual GIP/GLP-1 receptor agonist for metabolic support.

Overview

Tirzepatide is a dual GIP and GLP-1 receptor agonist. The compounded formulation is not FDA-approved. Brand-name tirzepatide (Mounjaro, Zepbound) is FDA-approved. Whether the compounded formulation produces equivalent results to approved products has not been independently studied.

Mechanism

GIP and GLP-1 receptors together influence insulin secretion, glucagon suppression, gastric motility, and appetite regulation through complementary pathways. Clinical trial data on brand-name tirzepatide showed meaningful effects on body weight and metabolic markers in the studied populations.

Clinical Considerations

• Same MTC/MEN2 contraindications as all GLP-1 class agents.
• Side effect profile similar to GLP-1 monotherapy.
• Prescription required. HSA/FSA eligible when prescribed for a qualifying condition.

Potential Side Effects

• Nausea and GI effects (vomiting, diarrhea, constipation) — similar profile to GLP-1 monotherapy.
• Injection site reactions.
• Rare: pancreatitis risk — same monitoring applies as GLP-1 class agents.
• Contraindicated in personal or family history of MTC or MEN2.
